Confidentiality & Privacy Policy
The law protects the relationship between a client and a Psychiatric Rehabilitation Program Staff and information cannot be disclosed without written permission.
It is the policy of Concentric Wholeness Solutions, L.L.C. to accept the responsibility to serve the mental health needs of people. In order for Concentric Wholeness Solutions, L.L.C. to accomplish this purpose, Concentric Wholeness Solutions, L.L.C. and its individual staff members must win the confidence and trust of Clients, and the public at large. Confidentiality means that the discussions, records, and treatment of, by or about any Client, shall be maintained with the right of the Clients privacy upheld as far as possible.
Exceptions include:
- Suspected child abuse or dependent adult or elder abuse, for which I am required by law to report this to the appropriate authorities immediately.
- If a client is threatening serious bodily harm to another person/s, I must notify the police and inform the intended victim.
- If a client intends to harm himself or herself, I will make every effort to enlist their cooperation in ensuring their safety. If they do not cooperate, I will take further measures without their permission that are provided to me by law in order to ensure their safety.
Discrimination Policy
Concentric Wholeness Solutions does not discriminate against any person on the basis of race, color, national origin, disability, financial status; (because the individual is unable to pay, because the payment for those services would be made under Medicaid), sexual orientation, or age in admission, treatment, or participation in its programs, activities, or in employment. No program or activity under the direction of Concentric Wholeness Solutions, L.L.C. shall exclude from participation, deny benefits to, or be subject to discrimination, any individual solely by any reason.
